Opt Out Form

The Cheatham County School District is partnering and participating in a student survey with the Cheatham County Community Enhancement Coalition.

The survey is designed to assess the prevalence of alcohol and drug use among students in grades 8, 10 and 12 as well as related attitudes and experiences.

The survey will be administered in the classrooms of students in grades 8, 10 and 12. It is completely anonymous, meaning the students’ names will not be associated with the survey data.

Participation in the survey is voluntary. The results of the survey will be used for school planning and prevention program development.

The district is participating in this important project because we believe young people’s use of these substances is a serious issue for our communities. We encourage you to support this project by agreeing to let your child participate in this survey.

A copy of the survey has been posted on the school district’s website under the heading “District News” as the “2017 Student Survey.”

Students will take the survey online on Nov. 20 and Nov. 21 in their ELA classroom.

According to Board Policy 6.4001, the parent/guardian may opt their child out of participation of this survey. The opt-out form is under “Related Documents.” Please return the opt-out form if you wish your student not participate in the survey.
